Participating in High School and Middle School Athletics
All students are required to have an annual physical and updated medical emergency information on file in the school’s Athletic Department, prior to the first official day of their sport practice.

All student-athletes must be registered in FinalForms in order to participate in school sports.  Once they have been registered, all medical forms are completed electronically in FinalForms. You only will need to register your student one time and the information will carry over from one school year to the next, however you will need to login in every year to update and re-sign the forms.  Both a parent/guardian AND the student-athlete are required to sign all forms (using different e-mail addresses). Once you have completed the forms electronically, download and print the OHSAA Pre-participation Physical Evaluation form and take to your doctor’s appointment (complete the first two pages prior to seeing the medical examiner).  After the medical examiner has signed and dated the form, you can either return a hard copy to the Athletic Department (or Middle School office for MS sports) OR you can now upload the completed signed form directly into your student’s FinalForms record.  Only after the signed OHSAA Physical Evaluation form is on file AND the other medical forms are signed in FinalForms, will the student be allowed to participate in sports.

Eligibility Requirements
Perry’s eligibility requirement is a minimum 2.0 GPA each nine weeks in order to remain eligible for all extracurricular participation. In addition, the “no pass, no play rule” continues to be in effect. Any failing grade will result in ineligibility for the next nine weeks. (See Student-Athlete Handbook.) Parents are encouraged to monitor interim reports carefully!
